To successfully complete your Diversity Visa (DV) Lottery application, you must provide accurate and complete information as outlined below. Any omissions or mistakes could make you ineligible for the DV Program:

1. Name

Provide your last/family name, first name, and middle name exactly as shown on your passport. If your passport only lists your first and last name, include only those. If you have only one name, enter it in the last/family name field.

2. Gender

Male or female.

3. Birth Date

Enter your date of birth in the format: day, month, year.

4. City of Birth

Specify the city where you were born.

5. Country of Birth

Enter the current name of the country where you were born, even if the name has changed since your birth.

6. Country of Eligibility

Your eligibility is based on your country of birth, not your current nationality or where you live. If your country of birth is not eligible, refer to the FAQ to check for alternate eligibility options.

7. Photographs

Submit recent digital photographs (taken within the last six months) of yourself, your spouse, and any eligible children. Ensure photos meet the U.S. visa standards. Failure to meet photo requirements or using previously submitted photos may disqualify your entry.

8. Mailing Address

Provide your full mailing address:

  • Legally separated

For a spouse, provide their name, birth date, gender, city/town, and country of birth, along with a photograph. Not listing an eligible spouse can disqualify you. If your spouse is a U.S. citizen or LPR, do not include them in your entry.

14. Number of Children

List all living, unmarried children under the age of 21, including:

  • Biological children
  • Legally adopted children
  • Stepchildren (even if they don’t live with you or will not immigrate with you).

Include their name, birth date, gender, city/town, and country of birth, along with a photograph. Omitting eligible children or listing someone who is not your child will disqualify your entry.
